Only the 50 states elect senators, so that is the answer the officer expects here.

Puerto Rico elects one member of the House for the whole territory, so there is a single answer. A resident commissioner takes part in the work of the House but does not vote on the floor; saying that Puerto Rico has no voting representative is also accepted.
Who is the governor of your state now?

The governor leads the executive branch of Puerto Rico, the way the President does for the country. USAGov keeps the current name at usa.gov.
What is the capital of your state?
San Juan
Puerto Rico is not a state, so the answer is the capital of the territory (Art. I, Sec. 4 (Sede del gobierno)).
